MIDI Off not always being caught

I'm on 1.1.7 of the SEM-V, and I built a custom midi pedal controller. I also play through a set of M-Audio interfaces (the Oxygen 61 and Axiom 25).

The axiom and oxygen work perfectly. But the custom controller can send note-off events (validated according to spec) and sometimes the SEM-V will hold the notes. It's not behavior I see with other virtual synths (that I've found) - it's JUST the SEM-V.

Is there a way I can help track down what is causing this somehow?

I *think* I see what the problem *might* be - I was sending MIDI off with velocity 127 (because it's not a pressure-sensitive) instrument. I changed it to send velocity 0, and the problem seems to be cleared up, although that only means I haven't found it again.
